*This membership form is to be filled and used by people who are 18 years of age or older, and only one form per person.

Due to scams and identity thefts, only Hmong State Headquarters is allowed to accept donations and process applications.  Anyone including staff and members who accept funds and process applications without approval from Hmong State Headquarters will be reporting to authorities and prosecuted with applicable laws.  Hmong State Anti-Communist Defense Funds is a non-profit political entity based in the United States with Federal Tax-Exempt status.
